For Patients
All patients are seen by appointment to minimize patient waiting time. To
enable a better selection of time and day, please call for appointments as early
as possible. Due to emergencies, there may be occasions when it will be
impossible for the doctor to keep a scheduled appointment. At such a time, our
office will make every effort to contact you about scheduling, if telephone
numbers are made available. Likewise, if you are unable to keep a scheduled
appointment, please contact our office as early as possible. This will provide
the opportunity for another patient in need of care to be seen. To avoid
charges, a twenty-four (24) hour notice is required.

appointment.
If you have questions concerning your insurance coverage, account status
and/or payment arrangements, you may contact the Bloomington Bone & Joint Clinic
Insurance/Billing Department directly at (812) 333-4600, Monday through
Thursday, 8:00am – 5:00pm and Friday, 8:00am – 3:45pm.
It is very important you understand your insurance policy. It is a contract
between you and your insurance company and is a means of reimbursing the
physician for services rendered. It is not a substitute for payment. We cannot
guarantee your insurance will cover any part of your claim.
Some insurance companies require a referral form before treatment can begin
by a Bloomington Bone & Joint Clinic physician. Referral forms are the
responsibility of the patient. A referral form may be obtained from your primary
care physician and can be faxed to our office at (812) 333-0611. If you are
unsure whether or not you need a referral, contact your insurance company. Their
telephone number should be located on the back of your insurance card.

- As a patient at Bloomington Bone & Joint Clinic, you have the right to:
- Be treated with courtesy, respect, and dignity, and have privacy
concerning your medical care.
- Receive a prompt and reasonable response to your questions and requests.
- Know the names of the Bloomington Bone & Joint Clinic physicians
responsible for your care and any other health care team members providing
medical services.
- Know what patient support services are available (including help with a
hearing impairment, or an interpreter in your language if you do not speak
English).
- Know what rules and regulations apply to your conduct as a patient.
- Participate in decisions that involve your care, including consideration
on ethical issues. You have the right to receive information about your
diagnosis, planned course of treatment, alternatives, risks, prognosis, and
outcomes of care that may impact your decisions regarding treatment.
- Accept or refuse any treatment except as otherwise provided by law.
- Be given upon request full information and counseling on financial
resources for your care.
- Receive a reasonable estimate of charges for medical care before your
treatment, if you have requested it. The actual costs may be higher based on
changes in your condition or treatment needs.
- Receive a copy of a clear and understandable itemized bill, and on
request, have the charges explained.
- Have impartial access to medical treatment regardless of your age, race,
religion, disability, sexual orientation, marital status or gender.
- Express grievances about any violation of your rights as stated in
Indiana law through the grievance procedure of the appropriate
state-licensing agency.
